Our Social Services department ensures the continuity of quality care and adequate patient/family support during your stay, as well as provide important transition services at the time of your discharge. We provide physical and psychosocial support to patients and family members coping with chronic, acute or terminal illnesses. We advise family caregivers and equip them with the skills and resources needed to receive maximum benefits from all available healthcare resources. We also arrange for discharge and post-hospital care for patients by assisting with the transition from the hospital setting to home health, rehabilitation, or long term care alternatives. We offer a wide variety of medical education and outreach services to groups and organizations, many of which are offered at little or no charge. These activities include:

For those 50 and older, we offer monthly networking and fellowship activities. These are offered at no charge to members and their guests. Senior Circle activities include guest speaker luncheons and breakfasts, movie matinees at the area senior center, old fashioned ice cream socials, holiday parties and more.
