Your Online Medical Tool

Our Patient Portal is your online electronic health document management tool. It provides a comprehensive view of your medical records that includes:

  • Test results
  • Procedures
  • Medications
  • Allergies
  • Immunizations
  • Other important items your provider(s) should know

To create your account: You will provide an email address at the time of your admission to the hospital. then you will receive an email invitation and step-by-step instructions of how to set up your account. No special equipment is needed to access this online tool. All you need is access to a computer, an Internet connection and the email address that you provided at the hospital registration desk.

After setting up your online account: You will be able to access your Patient Portal and print your complete medical history at Stone County Hospital. This Continuum of Care Document (CCD) allows you to share this information with your doctors and their staffs for any follow-up visits or care.

After each visit to our hospital, your account will be updated. Family and friends can access the information found on your portal, but only after you have given them permission. Hospital officials will give you the choice of giving an authorized representative access to your records during the registration process.
